精神障害者の社会復帰の阻害要因となるスティグマおよび看護師の職業アイデンティティに着目し， 精神科 （n=1917） 及び一般科 （n=676） に勤務する看護師を対象に質問紙調査を行なった。 その結果， 精神科群は一般科群に比してスティグマが有意に高く （p<0.01）， この原因としては， 投影同一化による患者の辛さの受容， 年齢および看護師経験年数の高さが考えられた。 職業アイデンティティについては両群で有意差がなかった。 一般科群は精神科看護について， 専門的で難しいと感じつつも， 精神科での経験は一般科での看護に活かせると考えていた。 一方， 精神科群では精神科看護のやり甲斐が十分に認知されていないと感じていた。 精神科群においてはスティグマと職業アイデンティティおよび収入や看護などへの満足度との間に有意な負の相関がみられたことから， 看護師のスティグマ解消のための啓発活動などの取り組みが必要であると示唆された。 |

Stigma is a serious factor that impedes the deinstitutionalization of persons with psychiatric disability. Thepurpose of this study is to investigate the stigma experienced by nurses and the professional identity ofnurses working in the psychiatric （n=1917） and other wards （n=676）. The method used in the study isquestionnaire investigation using a stigma scale and the professional identity scale for nurses （PISN）.The result revealed that the nurses working in the psychiatric ward: （denoted by“psychiatric”） experiencehigher stigma than those working in other wards （denoted by“other”）. The factor used for calculating thisdifference is the“projective identification for patient with mental illness,”“age （career as a nurse）,”and other background information. Although the PISN reveals no significant difference between the“psychiatric”and“other”groups, both groups consider psychiatric nursing to be“special”and“difficult.”Further, theyalso believe that the importance of psychiatric nursing is not sufficiently understood by all nurses. On theother hand, nurses in the“other”group believe that experience in psychiatric ward is useful in all otherwards. In the“psychiatric”group, a negative correlation was found between the significance of stigma andthe satisfaction with. Of care provided, income, and the hospital they work in. Therefore, further investigation is needed to clarify the role of stigma. |
